Mental Health Within a Family Medicine Framework

An enduring dimension of Family Medicine practice is engagement with mental health. Dr. Vieira approaches it not as a separate specialty, but as an integral part of comprehensive clinical care. This commitment appeared from the very beginning of his career. In his first year of practice as a family physician in 2000, he was already pursuing courses directly relevant to clinical encounters — including psychoanalysis and psychopathology related to eating disorders (2000), sexual identities from a psychological perspective (2001), and psychoanalysis and culture (2002). These were not peripheral interests. Rather, they were an early and deliberate effort to expand the clinical vocabulary available when facing presentations where biological, psychological, and relational dimensions intersect.

This orientation deepened over time. In 2004, Dr. Vieira completed a 68-hour in-person course in the foundations of psychoanalysis in clinical practice — not with the intention of becoming a therapist, but to develop a more structured perceptual framework for understanding individuals and families in clinical contexts. The psychoanalytic and systemic perspectives acquired through this training consolidated what had been growing since the start of his clinical work.

Mental health remained a consistent thread across both the clinical and managerial dimensions of Dr. Vieira’s career. The engagement that began in the consulting room — through psychoanalytic inquiry and attention to psychopathology — continued through his academic formation. When Dr. Vieira undertook his MSc in 2018, interest in mental health was not a new arrival. Instead, it was an established part of his professional identity. It informed both the analytical lens he brought to his studies and the direction of the post-graduation engagement that followed.

From Clinical Curiosity to Policy Engagement

Following the completion of his MSc, Dr. Vieira undertook a short-term project management role within NHS England in 2018. He worked at the central level with the Mental Health team in Operations and Information at Skipton House, London. This was a management and analytical engagement, not a clinical one. The work involved healthcare data on activity, costs, quality, and outcome measures. These informed commissioning decisions, benchmarking, and value-for-money assessments across mental health services. This experience gave him a systems and policy perspective on how mental health care is structured, resourced, and evaluated within a national public health system.

ADHD and Neurodevelopmental Care

This longitudinal investment in understanding the mind within medicine — from psychoanalytic theory to systemic family approaches and, more recently, to neuroscience and neurodevelopmental conditions — finds its current expression in ADHD care within a Family Medicine clinical framework. Dr. Vieira understands ADHD not as an isolated diagnostic category, but as a neurodevelopmental condition with broad clinical, behavioural, and relational dimensions. As a result, it requires exactly the kind of integrative clinical reasoning that Family Medicine is equipped to provide.

A Personal Note on Intellectual Formation

The intellectual trajectory described above is not incidental to clinical practice — it is constitutive of it. The decision in 2004 to study psychoanalytic theory as a clinician reflects a distinct way of approaching medicine. It involves curiosity about what lies beneath the presenting complaint. It also involves attentiveness to the relational dimensions of the clinical encounter. Furthermore, it requires an understanding that patients bring not only symptoms but also histories, meanings, and contexts.

This curiosity has continued in different forms.
