Dr. Fábio Luiz Vieira’s engagement with public health spans more than two decades. He has combined clinical practice, management, and teaching within Brazil’s Unified Health System (SUS). His trajectory includes frontline work as a physician within public health structures, and formal postgraduate training in public health management. Additionally, he developed and disseminated a community health methodology (territorialização) that served as a model for primary care practice in São Paulo. He also participated in medical recruitment processes at municipal and national level, and coordinated a home medical care programme for chronic conditions. This sustained commitment to public health is simultaneously clinical, managerial, educational, and research-oriented.

Clinical Work and Public Health Management in Brazil’s Unified Health System (SUS)

Within Brazil, Dr. Vieira’s engagement with public health extends well beyond individual clinical care. His trajectory includes work as a physician within public health structures and direct involvement in the management of public health projects. As a result, he brings both frontline clinical responsibility and a broader understanding of how health systems are organised, resourced, and evaluated.

This dual role — as a practising clinician and as someone engaged with the organisational and policy dimensions of public health — reflects the interdisciplinary orientation of his broader professional identity. Indeed, it is this combination of clinical and managerial experience that makes Dr. Vieira’s public health profile distinctive.

A concrete instance is his appointment as manager of the Vila Aparecida Family Health Unit, in the Pedreira district of southern São Paulo, where he was responsible for the day-to-day running of the service. He held the post from 2001 to 2002, overlapping with his postgraduate specialisation in primary care unit management at the University of São Paulo’s Faculdade de Saúde Pública — formal training and managerial practice running in parallel, in a unit serving a territory of high social vulnerability.

Public Health Congress Participation and Contributions

Throughout his early career, Dr. Vieira maintained an active presence in Brazil’s main public health congresses. He both presented original work and contributed as co-author. Furthermore, he engaged actively with the national academic community. These engagements spanned from the V Congresso Brasileiro de Saúde Coletiva (1997) to the VII Congresso Paulista de Saúde Pública (2001). Territorialização: Community Health Methodology and Academic Dissemination

Territorialização is a foundational methodology in Brazil’s Unified Health System. It involves the systematic study and mapping of a defined geographic territory — its population, social determinants, health needs, and available resources — as the basis for planning community health actions. Far from a bureaucratic exercise, it is a structured process for understanding the social fabric and health dynamics of a given area. Consequently, it underpins effective primary care delivery within the SUS.

Postgraduate Teaching in Territorial Health Assessment

Dr. Vieira developed his postgraduate thesis on territorialização during his Specialisation in Family Medicine at Unifesp — Universidade Federal de São Paulo, under Prof. Eleonora Menigucci. The thesis examined how this methodology applies in practice. Subsequently, it was implemented in a peripheral area of São Paulo as the foundational community health assessment for a new primary care unit.

Following this work, Dr. Vieira was invited to teach the territorialização process within postgraduate courses and other educational settings. He transmitted both the theoretical framework and the practical application of the methodology. Notably, this teaching role reflected direct recognition of his hands-on experience implementing the methodology in a real community health context.

Medical Recruitment Processes: Municipal and National Roles

As a medical manager of a primary health centre within the SUS, Dr. Vieira participated in multiple medical recruitment and selection processes. He contributed at both the São Paulo municipal health system level and to processes organised by the Brazilian Ministry of Health. Specifically, this involvement required clinical expertise and a clear understanding of the competency requirements for physicians working in Brazil’s public health system.

Home Medical Care Coordination: Chronic Conditions Programme

Dr. Vieira held a coordination role in a home medical care programme for patients with chronic conditions within the São Paulo municipal health system. In this capacity, he developed the clinical and operational criteria for patient admission to the programme. Specifically, these criteria determined which patients with complex or advanced chronic conditions would qualify for home-based care, and under what clinical and social circumstances. This work required integrating clinical reasoning with service design and resource management principles.

The criteria and outcomes developed through this coordination role were subsequently presented at public health scientific events. These presentations contributed to the broader evidence base on home care models within primary and community health settings in Brazil. Furthermore, this intersection of service development, clinical governance, and scientific communication illustrates the managerial dimension of Dr. Vieira’s public health practice.

Home care in the context of the Brazilian public health system is rarely a purely clinical matter. The patients served by the Campo Limpo Home Care Service (SAD) were predominantly elderly individuals with advanced chronic conditions, many living in peripheral urban areas with limited family support networks and significant social vulnerability. Determining eligibility for home-based care therefore required more than clinical assessment — it demanded an understanding of the patient’s living conditions, household capacity for informal caregiving, access to basic resources, and the social risks associated with inadequate follow-up. This social lens was built into the admission criteria Dr. Vieira helped develop, reflecting a model of care in which clinical complexity and social circumstance are evaluated together rather than separately.

Academic Engagement: Guest Lecturer at UNESP Botucatu

In 2024, Dr. Vieira contributed to medical education as a guest lecturer at the Faculdade de Medicina de Botucatu, UNESP — one of Brazil’s recognised medical schools. He presented on teleconsultation in primary health care, a topic of increasing relevance in the Brazilian health system. Moreover, this engagement reflects the direct connection between his current telemedicine practice and academic medical education.

Dr. Vieira’s public health work does not exist in isolation from his wider professional experience. For instance, his postgraduate studies at Imperial College London (MSc in International Health Management) and at the London School of Hygiene & Tropical Medicine provided a comparative academic framework.
